The proper form may be found under the section "School and District Updates," "School Forms," then select "Change Request for a CDS Code" (form TSD-02). This form must be submitted to the CDE per instructions.

Preloaded Data
The Name of School and CDS Code is retrieved from the School Student Counts data entry form.
The number 1, 2, or 3 in Grade Span Group column identifies the grade-span designation for each school. This designation has been assigned by the CARS on the basis of the information in the LEA's enrollment data and Free and Reduced Price Meals Program report. Grade-span 1 is for elementary grades, grade-span 2 is for middle grades, and grade-span 3 is for high schools.
The Projected Enrollment data was entered on the School Student Counts screen.
The Projected Low Income Students data was entered on the School Student Counts screen.
The CARS will calculate the Low Income Percent.
Eligible: Indicates whether the school is Eligible for Title I, Part A funding. A school is eligible if the school’s low income percent is equal to or greater than district low income percent.
Funding Required: Indicates whether Title I funding is required for this school. Funding is required if the school’s low income percent is equal to or greater than 75 percent.
The Ranking is only used to determine eligibility for Title I funds.
Note: This data collection will not display if the LEA did not apply for Title I Part A.
Note: Additional Information on grade-span option: The Educational Data Management Division within the CDE has established the following designations of grade-spans based upon the grades served by a school.
