Instructions: Title I School PI Activities

Consolidated Application, Spring Release, Phase II instructions for the 2011-12 Title I, Part A, School Program Improvement Activities.
Data Collection Purpose

Per ESEA Section 1116, LEAs must report school level Program Improvement (PI) activities related to Title I, Part A during the 2011-12 school year. This includes, but is not limited to, LEA provision of public school choice transportation costs and supplemental education services for students in PI schools and PI compliance activities based on the number of years a school is in PI status.

Procedures
Step Action Program Instructions
1 Enter the Number of students in this PI school who applied for public school choice provisions of ESEA Section 1116. Enter the number of students in this PI school (required for PI years 1-5 schools) who applied for public school choice provisions of ESEA for the 2011-12 school year. Note: Non-participation of eligible students in the provisions of ESEA public school choice will be reviewed by CDE program staff to determine reasonableness.
2 Enter the Number of Students who Transferred under ESEA. Enter the number of students from this PI school who transferred and attended a non-PI school under the ESEA public school choice and transportation provisions during 2011-12. This number is cumulative to all students from this PI school who transferred and attended during 2011-12 a non-PI school under ESEA public school choice provisions.
3 Enter the Number of Students who Transferred under Local or State School Choice Programs. Enter the number of students otherwise assigned to this PI school during 2011-12 who transferred and attended a non-PI school under a local or state school choice program, such as open enrollment. This is a cumulative number of all students from this school who transferred and attended a non-PI school under a local or state school choice program.
4 Enter the Number of Students who applied for SES. Enter the number of students in this PI school (required for PI years 2-5 schools) who applied to receive the ESEA provision for supplemental educational services (SES) during 2011-12. (The LEA may voluntarily opt to provide SES in PI year 1 schools.) Note: Non-participation of eligible students in SES programs will be reviewed by CDE program staff to determine reasonableness.
5 Enter the Number of Students who Received SES. Enter the number of students in this PI school who received SES in 2011-12.
6 In the PI Activities Exception Comment box, provide an explanation of why student counts are zero. Provide details regarding a zero student count for the provision of ESEA requirements for PI schools.
7 Enter any Additional Comments. Optional: provide additional information for CDE review and/or for LEA records.

For every school in the LEA that is in year 3 of PI during 2011-12, the LEA implemented at least one of the following corrective actions (check all that apply):

1 If school staff were replaced due to a failure to make adequate yearly progress (AYP), check the Replaced School Staff box. Action taken pursuant to ESEA Section 1116(b)(7).
2 If a new curriculum, including appropriate professional development, was implemented, check the Implemented a New Curriculum box. Action taken pursuant to ESEA Section 1116(b)(7).
3 If management authority was decreased, check the Decreased Management Authority box. Action taken pursuant to ESEA Section 1116(b)(7).
4 If an outside expert was appointed to advise the school on making AYP based on its school plan, check the Appointed an Outside Expert box. Action taken pursuant to ESEA Section 1116(b)(7).
5 If the school year or day was extended, check the Extended School Year or Day box. Action taken pursuant to ESEA Section 1116(b)(7).
6 If the internal organization was restructured, check the Restructured box. Action taken pursuant to ESEA Section 1116(b)(7).
7 Provide a description of internal organizational restructuring activities. Action taken pursuant to ESEA Section 1116(b)(7).

For a PI school in year 4 and beyond, the LEA develops and implements an alternative governance plan to restructure the school’s governance and staffing using at least one of the approaches (check all that apply).

1 If there is a plan to reopen the school as a charter school, check the Plan to Reopen box. Alternative governance approach that the LEA developed for the school pursuant to ESEA Section 1116(b)(8).
2 If there is a plan to replace all, or most of, the staff, check the Plan to Replace box. Alternative governance approach that the LEA developed for the school pursuant to ESEA Section 1116(b)(8).
3 If the LEA entered into a contract with an entity, such as a private management company with a demonstrated record of effectiveness, to take over the operation or administration of the PI public school, check the Contracted box. Note: Do not check this box, for example, if the LEA contracted with a county office of education or other entity to provide technical assistance and support.
4 If there is a plan to institute major restructuring activities, check the Institute box. Alternative governance approach that the LEA developed for the school pursuant to ESEA Section 1116(b)(8).
5 Provide a description of restructuring activities. Provide a detailed description of the plan and progress to institute major restructuring activities pursuant to ESEA Section 1116(b)(8).

School: is in PI Year 2 or higher so Applied for SES must be equal to or greater than zero. If the school is in PI year 2 or beyond, SES student count must be equal to or greater than zero.
School: The number of students who received SES cannot be greater than the number of students who applied. The number of students who received SES cannot be greater than the number of students who applied.
School: The number of students who received Choice cannot be greater than the number of students who applied. Total students transferred under the provisions of ESEA cannot be greater than the number of students who applied.
School: Must provide an explanation why no students apply for or received Choice and or SES. Sum of all counts can not be equal to zero. If zero, the LEA must provide comments explaining the zero value.
School: Number of students who Applied for Choice must be equal to or greater than zero. Applied student count for public school choice provisions of ESEA must be equal to or greater than zero.
School: Must identify one or more corrective actions implemented. If the school was designated PI year 3 or more, at least one activity must be selected.
School: A description of internal organizational restructure activities is required. If internal restructuring is checked, the restructuring activities description is required.
School: a description of planned restructure activities is required. If major restructuring planned is checked, the restructuring activities description is required.
School: A description of major restructuring activities instituted is required. If major restructuring was instituted is checked, the restructuring activities description is required.
