Data Collection Purpose
This form provides notification to CDE of a school's eligibility and local board approval to operate under and report as SWP.

Guidance for the ESEA Title I, Part A SWP Authorization can be found on the SWP Web page and in Section 1114, Title I, ESEA.
Please note: a re-evaluation of the SWP as result of the required annual review does not require a new notification of authorization of SWP.
Information must be provided only for schools being designated as SWP.

| 1 | Enter a check in the box Authorized as a schoolwide program. | For any authorized school(s) enter a check in the box Authorized as a schoolwide program. The CARS will prepopulate the Authorized box for those schools for which the LEA has previously notified CDE of the authorization. |
| 2 | Enter the Local Board Approval Date in mm/dd/yyyy format. | Provide the Local Board Approval Date. The CARS will migrate the local board approval date for those schools for which the LEA has previously notified CDE of the date.
Error Message: The Local Board Approval Date can not be a future date or a pre-NCLB approval date. Resolution: Ensure that the Local Board Approval Date is correct. |
| 3 | Enter the SIG Approval Date in mm/dd/yyyy format, if applicable. | Provide the SIG Approval Date, if applicable. The CARS will prepopulate the SIG Approval Date for those schools for which the LEA has previously notified CDE of the date.
Error Message: The SIG Approval Date can not be later than Local Board Approval Date if Poverty Level Percent is less than 40. Resolution: Ensure that the SIG Approval Date is correct. |
| 4 | Enter the Poverty Level Percent. | If the Poverty Level Percent is lower than 40, there must be a SIG Approval Date.
