Consolidated Application, Spring Release program instructions for the 2012-13 Title I, Part A, Nonprofit Private School Student Demographics and Services.
Data Collection Purpose

The 2012–13 Title I, Part A Nonprofit Private School Student Demographics and Services Report provides demographics and services for nonprofit private school students designated as receiving Title I, Part A equitable services.

This data collection will only be available for LEAs that participate in Title I, Part A.

Guidance

Title I, Part A School Student Demographics and Services data collection requests information in response to Public Law 107-110 (Section 1501); the CDE has developed this form to collect data on students served by compensatory education programs in public schools and to submit that information annually to the U.S. Secretary of Education.

This form is to be completed for each private school that received ESEA, Title I, Part A funding during 2012–2013. Information must be reported for those students who received Title I, Part A services. The LEA has the responsibility for reporting all information for all schools by the due date.

To provide the most accurate total of students served, information collected is for the numbers of students served in each of the identified categories during 2012–2013.

2 Enter the American Indian or Alaskan Native student count. Enter the number of American Indian Or Alaskan Native students who received Title I, Part A services during 2012–13.
3 Enter the Asian student count. Enter the number of Asian students who received Title I, Part A services during 2012–13.
4 Enter the Black or African American student count. Enter the number of Black or African American students who received Title I, Part A services during 2012–13.
5 Enter the Native Hawaiian or Other Pacific Islander student count. Enter the number of Native Hawaiian students who received Title I, Part A services during 2012–13.
6 Enter the White student count. Enter the number of White students who received Title I, Part A services during 2012–13.
7 Enter the Two or More races student count. Enter the number of Multiracial students who received Title I, Part A services during 2012–13.

1 Enter the Migrant student count. Enter the total number of migrant students with certificates of eligibility who received Title I, Part A services during 2012–13.

Error Message: School: the sum of migrant students cannot be greater than the sum of Male and Female students.

Resolution: Enter data to ensure that the sum of migrant students is not greater than the sum of male plus female students.

2 Enter the Students with Disabilities student count. Enter the total number of disabled students with IEPs who received Title I, Part A services during 2012–13.

Error Message: School: the sum of students with disabilities cannot be greater than the sum of Male and Female students.

Resolution: Enter data to ensure that the sum of students with disabilities is not greater than the sum of male plus female students.

1 Enter the Received Mathematics Services student count total. Enter the unduplicated count of the total program participants in mathematics for all students who received Title I, Part A services during 2012–13.

Error Message: School: the sum of Received Mathematics services students cannot be greater than the sum of Male and Female students.

Resolution: Enter data to ensure that the sum of Received Mathematics services is not greater than the sum of male plus female students.

2 Enter the Received Reading Language Arts Services student count total. Enter the unduplicated count of the total program participants in reading language arts for all students who received Title I, Part A services during 2012–13.

Error Message: School: the sum of Received Language Arts services cannot be greater than the sum of Male and Female students.

Resolution: Enter data to ensure that the sum of Received Language Arts services is not greater than the sum of male plus female students.
