A sponsor representative must contact the Internal Revenue Service (IRS) by calling 877-829-5500 or by sending a fax to 513-263-4330. The IRS will update the information by telephone and then fax the sponsor a revised IRS Determination Letter the same day. Sponsor representatives may contact the IRS by mail at the following address:

Once the sponsor has received the IRS Determination Letter, a representative may fax it to the Summer Food and Data Management Unit (SFDMU) of the Nutrition Services Division at 916-445-4842 or mail it to the California Department of Education address provided below.

A sponsor representative must submit the changes on agency letterhead to SFDMU of the Nutrition Services Division by fax at 916-445-4842 or mail it to the California Department of Education address provided below.
All Agencies
In addition to the above documentation sponsors must fill out a Payee Data Record STD 204 form (PDF; Outside Source).
The STD 204 must be mailed to the address provided below and contain the original wet signature of the sponsor representative:
