SNP Address Changes

Procedures to submit corrections for School Nutrition Program (SNP) address changes.

Private, Non-Profit Agencies, Charters

An authorized school/agency (sponsor) representative must contact the Internal Revenue Service (IRS) by calling 877-829-5500 or by sending a fax to 513-263-4330. The IRS will update the information by telephone and then fax the sponsor a revised IRS Determination Letter the same day. Sponsor representatives may contact the IRS by mail at the following address:

Once the sponsor has received the IRS Determination Letter, a representative may fax it to the School Nutrition Programs Unit (SNPU) of the Nutrition Services Division at 916-445-5731 or mail it to the California Department of Education address provided below.

Public Agencies

An authorized sponsor representative must submit the changes on agency letterhead to SNPU of the Nutrition Services Division by fax at 916-445-5731 or mail it to the California Department of Education address provided below. On school letterhead, please state the old address and the request to change to the new address. The letter must be signed by the authorized school official.

All Agencies

  • Please complete the Payee Data Record STD 204 form External link opens in new window or tab. ; accepted with original signature only.
  • Update the appropriate school directory with your new address for your school/agency type: CA School Directory, Charter School Directory, or Private School Directory.

Once you have completed the appropriate changes noted above for your school/agency type, you may submit the STD 204 by mail, and fax or e-mail supporting documentation to the School Nutrition Programs Unit (SNPU) County Program Specialist. You may contact the SNPU at 800-952-5609 to locate your SNPU County Program Specialist or if you need further assistance with this process.
