(Required for all Applicants)

2012–13 General Assurances and Certifications

(Do not submit as part of the application.)

All sub-grantees are required to retain on file a copy of these general assurances for the charter school records and for audit purposes. Please download the 2012–13 General Assurances and Certifications located on the California Department of Education (CDE) Funding Forms Web page [http://www.cde.ca.gov/fg/fo/fm/ff.asp]. Do not submit this form (PCSGP Form 9) or the General Assurances and Certifications printout to the CDE. The applicant is required to print and retain a copy of these specific assurances at the charter school site. The signature on the front of the application indicates acknowledgement and agreement to all assurances.

Certifications Regarding Drug-Free Workplace, Lobbying, and Debarment and Suspension

(Do not submit as part of the application.)

Download the following three forms from the certifications section on the CDE Funding Forms Web page [http://www.cde.ca.gov/fg/fo/fm/ff.asp].

  1. Drug-Free Workplace
  2. Lobbying
  3. Debarment and Suspension

Print, sign, and retain at the charter school site. The signature on the front of the application indicates acknowledgement and agreement to all assurances and certifications.
