Forms And Other Information
Document Description
Grant Award Notification (AO-400)
See Guidelines for Sample
The Grant Award Notification (AO-400) contains funding information for the EETT Formula grant and must be signed and returned to the California Department of Education (CDE) in order to receive EETT funding for the year. To reject the grant award, write "DECLINE" on the AO-400, sign and return to the CDE within ten days of receipt of AO-400.
Assurances and Certifications
Download and keep on file
Assurances and Certifications are requirements of applicants and grantees as a condition of receiving funds. Applicants do not need to sign and return the general assurances and certification with the application; instead, they must download them and keep them on file to be available for compliance reviews, complaint investigations, or audits. Applicants that participate in the Consolidated Application - ConApp - should already have a copy of the general assurances and certifications on file and do not need to keep a separate copy. Program-specific assurances are not included here but are listed separately in the Guidelines (see page 13).

End of Period Expenditure Report
See Guidelines for Sample

End of Period (EOP) Expenditure Report can be filed with CDE when the EETT Formula funds have been fully expended and/or obligated but no later than September 30, 2012. Upon submission and approval of the EOP, the grant award amount less any reported adjustments will be reimbursed to the LEA.
The EOP and filing instructions will be posted on our listserv. Please sign up to join our Lyris Listserv by simply sending a "blank" message to join-eett-formula@mlist.cde.ca.gov.
