This Management Bulletin transmits new documentation requirements for children enrolled in child care centers (see exceptions) and family day care homes. The requirements do not apply to outside-school-hours centers; after-school, at-risk centers; emergency shelters; and adult care centers.

New federal regulations require sponsors to ensure that a child's enrollment document must be signed by a parent or guardian, be updated annually, and contain the following information:

  • The child’s normal days and hours of child care; and
  • The meals that the child ordinarily would receive during normal hours of care.

All child care centers (with the exceptions noted above) and day care homes must have this information on file for each enrolled child. This requirement has two implementation deadlines:

  • April 1, 2005: If a center or home enrolls a child on, or after April 1, 2005, that child’s initial enrollment form must contain the required information.
  • September 30, 2005: If a child was enrolled prior to April 1, 2005, the center or home must collect the necessary information by September 30, 2005.

REMEMBER: The enrollment forms with the required information must be updated annually.
