Reference: Child and Adult Care Food Program Second Interim Rule: Title 7, Code of Federal Regulations, Part 226, Sections 15(e)(2)(3) and 18(e) and U.S. Department of Agriculture CACFP Policy Memo 05-03

This Management Bulletin transmits new documentation requirements for children enrolled in child care centers (see exceptions) and family day care homes. The requirements do not apply to outside-school-hours centers; after-school, at-risk centers; emergency shelters; and adult care centers.

New federal regulations require sponsors to ensure that a child's enrollment document must be signed by a parent or guardian, be updated annually, and contain the following information:

All child care centers (with the exceptions noted above) and day care homes must have this information on file for each enrolled child. This requirement has two implementation deadlines:

REMEMBER: The enrollment forms with the required information must be updated annually.
