Instructor Certificate Change Form (DOC)
This form is to be used by an instructor who is requesting an upgrade or limitation removal. In addition, please use this form to notify of any change to your special certificate (i.e. Automatic Only, Restriction 6, etc.)

It is imperative that instructors notify the California Department of Education when their information changes. This form is to be used when updating the following: name, home address, or employer change. In addition, please use this form to inform the department if you have retired, become independent, or are no longer active.
