Purpose

This Management Bulletin (MB) informs existing California Department of Education (CDE), Child Development Division (CDD), center-based, Family Child Care Home Education Network (education network) contractors, and California School-Age Families Education (Cal-SAFE) child care program coordinators of the requirements regarding the annual Program Self-Evaluation due on June 3, 2013. This MB rescinds and replaces MB 12-03.

In accordance with the CDE Desired Results System, the attached forms must be completed and submitted for each center-based contract type, education network serving children in California Center-based (CCTR), California Family Child Care (CFCC), and California Migrant (CMIG) contract types, and/or Cal-SAFE child care programs to the CDE by or no later than Monday, June 3, 2013, at 5 p.m. For Cal-SAFE child care programs that do not have CDD contracts, all references to the "contractor" or “contract” will refer to the local education agency.

For fiscal year (FY) 2012–13, “age group” will be defined as infant/toddler and preschool-aged children. For this year only, the Program Self-Evaluation Annual Report will not require submission of information for school-age children, unless school-age children are the only ages served by the contractor.

Note: The Personnel Roster and Attestation forms will be submitted at a later date.

Alternative Payment, Resource and Referral, and CalWORKs Stage 2 and Stage 3 contractors are not required to complete the Desired Results Annual Program Self-Evaluation for FY 2012–13.

Statutory or Regulatory Requirements                     

The Desired Results Annual Program Self-Evaluation and requirements are described in California Code of Regulations, Title 5, sections 18270.5, 18279, 18280, and 18281.

Program Self-Evaluation Submission Requirements

The complete Desired Results Annual Program Self-Evaluation consists of: (1) Instructions for Completing the FY 2012–13 Program Self-Evaluation (DOC) and (2) FY 2012–13 Program Self-Evaluation Forms (DOC). Electronic submission of the Program self-Evaluation is not acceptable. Mail only the original of all documents for each contract type by age group, education network by age group, and/or Cal-SAFE child care program to the CDE by or no later than Monday, June 3, 2013, at 5 p.m. to the following address:
