It is recommended you review these instructions prior to completing the Annual Information Survey. Additionally, help links located throughout the online survey form will link to these instructions.
Upon entering the survey, you will be asked to find your county, authorizing local educational agency and school. You will then enter your personal identification number (PIN). Verify the information is accurate, correct if needed and enter any information that is not populated on the form. Once you hit the submit button you will be unable to re-enter. If you feel you have any corrections to be made after submission, contact the Charter Schools Division at Charterdata@cde.ca.gov.
