Upon entering the survey, you will be asked to find your county, authorizing local educational agency, and school. You will then enter your personal identification number (PIN). Verify the information is accurate, make corrections if needed, and enter any information that is not populated on the form. Once you hit the submit button, you will be unable to re-enter the survey. If you feel you have any corrections to be made after submission, contact the Charter Schools Division by e-mail at firstname.lastname@example.org.
