- Open the CASEMIS Software
- Click Reports – (located on the bar across the top of the screen)
- From the “Reports” screen, select “Enrollment Full Report”
- From the “Enrollment Full Report” screen, under the section “Default Listing Reports” (lower right corner), select Annual Service Plan
- From the “Multi Selection Report” screen, under “Data File and Date”
Select “Browse” and locate your Student Data (Table A)
Select “Browse” and locate your Service (Table B) - Select “Preview” (located on the bar across the bottom of the screen)
- The Annual Service Plan (the following forms will open, be completely filled-in, and sorted by LEA)
Form ASP-02a, Annual Service Plan
Form ASP-02c, Site Name and Type of Facilities
Form ASP-02d, Infant Services
Form ASP-02e, Pre-School Services - Select “Print” to print the Annual Service Plan Location Report.
At this point, two options available:
Option 1Make any corrections directly on these forms.The most common error may be ***Sch Code Not found***. Correct this error by entering the correct data – it can be hand written.Submit these forms to CDE.
Option 2
At the “Multi Selection Report” screen, select “To Excel” (located on the bar across the bottom of the screen) to download the data to an excel spreadsheet.
Make any corrections on the excel spreadsheet.
Retype the data onto the appropriate forms that have been downloaded.
Submit these forms to CDE.
IF YOU DOWNLOAD TO AN EXCEL SPREADSHEET, the CASEMIS software does not allow the user to make corrections to the data on the Excel spreadsheet. Copy the corrected data back into the CASEMIS software, and then print corrected Service Plan forms. However, you may want to use this report to ensure that these errors are corrected prior to submitting the June 30 CASEMIS data.
