Supplemental Educational Services (SES) Provider

SES Provider Application Submission for Cohort 2015

Applications to become a state-approved SES provider are now being accepted for the 2015-2017 approval period. The deadline for the submission of all required materials is 5:00 PM PST on October 30, 2014.

Logon

If you have already saved the provider profile online, please enter your confirmation number and select the "Logon" button to retrieve your profile. Otherwise, click the link "Submit New SES Provider Profile" by following the instructions below to complete the profile.

The profile has two parts and must be submitted according to the following instructions:

Part A - Applicant Contact Information

Complete Part A and select the "Save" button ONLY (at the end of the page). Please do not select the "Submit" button until you have completed Part B of the profile. Please note that once the profile is submitted, no changes can be made. You will receive a confirmation number after you save Part A. Please make note of the confirmation number that you receive. Use the confirmation number to go back and edit and complete the profile.

Part B - Service Area(s) and Program Design

Complete Part B and click the Save button. Use the confirmation number assigned to go back and edit the profile. Do not submit the profile until it is complete and no changes are needed. Once the profile is submitted, no changes can be made unless an e-mail is sent to cdavis@cde.ca.gov to request your profile be reopened.

Submit the entire profile (Parts A and Part B) before 5:00 PM PST on October 30, 2014. The profile should be submitted online before the deadline of October 30, 2014 to ensure a copy can be included with the two hard copies of the complete application that is to be received at the California Department of Education (CDE).

Print, sign and date the printed profile. Use the printed profile with the confirmation number printed on the top as the cover to each of the two complete sets of the application materials (narrative responses and supporting documents) delivered to CDE by 5:00 PM PST on October 30, 2014.

Please note applications without confirmation numbers will not be accepted.
Step 2: Upload Narrative Response

All applicants must upload the narrative response (Section III, Part C) by using the confirmation number received from saving or submitting the profile. Use the confirmation number to logon to the profile page and go to Step 2: Upload Narrative Response. All electronic copies must be received by the submission deadline. If there are multiple submissions, the CDE will only use the latest version of the application. All applicants will upload narrative responses by following the instructions below.

  1. Narrative responses described in Section III of the RFA must be typed by using the template provided on the SES RFA Web page at http://www.cde.ca.gov/fg/fo/r16/ses11rfa.asp.
  2. If the applicant has a Microsoft Word version higher than 2003, please save the document as a lower version before uploading in order for CDE to read the file.
  3. In the header section of the narrative response, provide the organization name and the confirmation number of the submitted profile.
  4. Print the narrative responses to be included in the application package.
