The UCP Checklist is a comprehensive list of required statements to be included in the six different Uniform Complaint Procedures (UCP) policies, procedures, notices and forms according to the California Code of Regulations, Title 5, sections 4600-4687. - LEA Self-Certification of UCP 3 (PDF; Updated 28-Nov-2011)
The Self-certification for UCP 3 is a form provided by CDE for the LEA to complete before its Federal Program Monitoring (FPM) On-line Review declaring either (a) the LEA had no UCP complaints within a 12-month period or (b) the proper implementation of the Uniform Complaint Procedures (UCP) investigation process, consistent with the requirements of 3.1, 3.2. The “Sample Complaint Log” is a tool designed to assist LEAs prepare for the online review of UCP 3. The use of this sample log is not required. However, LEAs are required to provide the CDE information about the complaints received in the past 12 months in order for the Department to complete its review of UCP 3.

The Self-certification for UCP 5 is a form provided by CDE for the LEA to complete before its Federal Program Monitoring (FPM) On-line Review declaring the proper implementation of the Uniform Complaint Procedures (UCP) process, consistent with the Williams Complaints requirements found in 5.0 of the UCP Instrument. The LEA will upload the self-certification document into CAIS.
