CAHSEE Waiver & Exemption Data Collection Form

Instructions for using the CAHSEE Waiver and Exemption Data Collection Form as Required by Title 5 Regulations sections 1207.1 and 1207.2 and California Education Code Section 60851.
   

This form is provided to local educational agencies (LEAs) to collect data for those students with disabilities who received a California High School Exit Examination (CAHSEE) waiver or exemption pursuant to California Education Code (EC) sections 60851 (c) or 60852.4.

LEAs are required to report the data under California Code of Regulations, Title 5 sections 1207.1 and 1207.2. The California Department of Education (CDE) has developed an encrypted electronic form to be used by LEAs to report the data.

Please follow the instructions below for completing each of the fields in the CAHSEE Waiver and Exemption Data Collection Form.

The form can be found on the on the CAHSEE Web page:

  1. When prompted for a password to access the form, enter the password you received via e-mail.
  2. Pursuant to Education Code sections 60851(c) or 60852.4, complete each of the fields for those students with disabilities in the Class of 2007 that:
    • Have been granted a waiver to one or both parts of the CAHSEE requirement, and/or
    • Have an IEP or Section 504 plan dated on or before July, 2006, that indicates that the pupil is scheduled to graduate in 2007, but who has not yet passed both sections of the examination.
  3. Each of the fields can only be populated with the codes described in the table below. Additionally, drop down menus have been created to aid in entering the data.
    1. To utilize the drop down menus, first select the cell for which you wish to enter data.
    2. Next click the downward facing arrow on the right side of the cell, which will generate the list of acceptable values.
    3. Then click the appropriate value of the list.

Field Name

Instructions for Completing Field

District Name

Enter the name of the district, county office of education, or charter school completing this form.

CDS Code

Enter the CDS code of the district, county office of education, or charter school completing this form. Enter the CDS code as XX-XXXXX-XXXXXXX.
For assistance in identifying your CDS code, go to the California School Directory Web page.

Student ID

Enter each student’s SSID. 
Note: This field is for local use only and will be removed once the files are collected by CDE.

Pupil’s Last Name

Enter the pupil’s last name.
Note: This field is for local use only and will be removed once the files are collected by CDE.

Pupil’s First Name

Enter the pupil’s first name.
Note: This field is for local use only and will be removed once the files are collected by CDE.

School Name

Enter the name of the school or charter school completing this form.

Primary Disability Code

Enter the student’s primary disability code in this field. The valid codes are listed below:
000- Student Receives no Special Education Services
210- Mental Retardation (MR)
220- Hard of Hearing (HH)
230- Deaf (DEAF)
240- Speech or Language Impairment (SLI)
250- Visual Impairment (VI)
260- Emotional Disturbance (ED)
270- Orthopedic Impairment (OI)
280- Other Health Impairment (OHI)
290- Specific Learning Disability (SLD)
300- Deaf-Blindness (DB)
310- Multiple Disabilities (MD)
320- Autism (AUT)
330- Traumatic Brain Injury (TBI)

Percent of Time in General Education

Provide an estimate of the percent of time the student is in general education using the ranges described below:
90 to 100 percent
51 to 89 percent
20 to 50 percent
Less than 20 percent

Did Pupil Receive a 2007 Diploma?

Indicate “yes” if the pupil received a diploma in 2007.

Indicate “no” if no 2007 diploma has been granted.

If Pupil Received a 2007 Diploma, Indicate if it Was a Result of Having Been Granted a Waiver or an Exemption?

Indicate “Waiver” if the pupil received a diploma in 2007 pursuant to Education Code Section 60851(c).

Indicate “Exemption” if the pupil received a diploma in 2007 pursuant to Education Code Section 60852.4.

  1. After entering the data into the form, please save the form as “Your District* CAHSEE Waiver and Exemption Data Collection Form.xls”. 
    *Note: Replace “Your District” in the above Excel file name with the name of your own district, for example, “LA Unified CAHSEE Waiver and Exemption Data Collection Form.xls”.
  2. Please submit all data via e-mail to cahseedatacollections@cde.ca.gov by February 29, 2008.
