Required: 2022–23 FFVP Midyear Assessment Survey


The 2022–23 Fresh Fruit and Vegetable Program (FFVP) grant year has reached the midpoint. The FFVP Team would like to thank you for your efforts in implementing the program in your school(s). As a condition of accepting the 2022–23 FFVP grant funds, you agreed to submit an FFVP mid-year progress report. This report will be completed and submitted using an online survey.

Deadline

You must complete the 2022–23 FFVP Midyear Assessment Report by Thursday, March 30, 2023, at 5 p.m.

The 2022–23 FFVP Midyear Assessment Survey is mandatory. A lack of response by the due date may jeopardize future FFVP funding.

Instructions

Complete and submit only one report for your school food authority (one report per grantee). If you are a school district with multiple schools participating in the FFVP, please submit one report capturing the FFVP implementation for all schools. The single report can provide specific information for activities that have occurred at each school.
