Draft Request for Applications (DOC)
This revised Draft Request for Applications (RFA) includes general information, forms, checklist and rubric. The application closed on Friday, March 14, 2014, at 4:00 p.m.
Forms 4-5-Budget Forms (XLS) This revised Excel workbook contains templates for Forms 4a, 4b, 5a, and 5b, the Budget Summaries and Budget Narratives. Only one workbook should have been submitted and must have included all sites, including the LEA.
Form 8-Sustainability of Reforms (XLS) This Word document contains Form 8: Sustainability of Reforms After the Funding Period Ends. This document was required for the application.
Form 10-Implementation Charts (XLS) This Excel document contains Form 10: Implementation Charts for each of the models. This document was required for the application.
Form 11-Tier III Implementation Charts (DOC) This Word document contains Form 11: Implementation Chart for a Tier III School. This document was required for Tier III schools only.

Certifications and Assurances

Document Description

General Assurances and Certifications

Required as a condition of receiving funds. Applicants do not need to sign and return them with the application; they must be downloaded and kept on file for compliance reviews, complaint investigations, or audits. Applicants that participate in the Consolidated Application should have a copy on file and do not need another copy. Program-specific assurances are not included here and should have been listed separately on the RFA.
