Universal Meals School Food Authority Survey


To help understand and assess the implementation of the California Universal Meals Program, and in follow up to the survey conducted in February 2022, the California Department of Education, in partnership with the Nutrition Policy Institute, is conducting a statewide formative evaluation survey.

Approximate Length: 30 minutes

To assist you with completing this survey, a reference copy of the survey is located in the download forms section of the Child Nutrition Information and Payment System External link opens in new window or tab. and is titled Online 2023 FSD Survey.

The goal of this survey is to capture details about your universal meals operations, and your successes, concerns and experiences.

The survey will ask you to agree to participate prior to answering any questions. Your participation is voluntary and will in no way affect your meal program. We value your input and your responses will be kept confidential and not linked to your name or school in any reporting.
