Health and Safety Supplies Needs Assessment Survey

In order to assist our sponsoring agencies who are serving their community during this health crisis, the California Department of Education (CDE) Nutrition Services Division (NSD) is assessing the emergency supply needs for program operators who have been providing meals during the COVID-19 unanticipated school closures.

The CDE wants to take every opportunity for you to receive the health and safety supplies you need as soon as possible, so please complete the survey by May 22, 2020.

Once we receive all the information, the CDE will use the survey responses to communicate local needs at the state level, and to work in partnership with the Governor’s Office of Emergency Services (Cal OES) to identify effective strategies for meeting those needs. Your survey response does not constitute an equipment or supply “order;” rather, provides the CDE with the information needed to ensure local provider needs are part of state considerations with regard to deploying and purchasing emergency supplies.
