Dear Foster Youth Services Coordinating Program Coordinators:

2015–16 FOSTER YOUTH SERVICES COORDINATING PROGRAM
YEAR-END EVALUATION REPORT

Each Foster Youth Services Coordinating (FYSC) Program that received FYSC Program grant funding for fiscal year 2015-16 is required to complete and return the FYSC Program Year-End Evaluation Report.

This report will meet the FYSC Program grant requirement to submit a report to the State Superintendent of Public Instruction (SSPI) at the end of each even-numbered year addressing the coordination and delivery of services, collaboration between partners, and quantitative data on program activities and services as well as student accountability measures for foster youth. This report will provide essential information necessary for compliance with California Education Code Section 42923, which requires the SSPI to submit a report to the Governor and the Legislature on the effectiveness of FYSC Program services provided to foster youth. A copy of this report form can be found on the California Department of Education Foster Youth Services Web page at http://www.cde.ca.gov/ls/pf/fy/.

Please use the attached FYSC Program Evaluation Tool to complete the Year-End Evaluation Report. The completed FYSC Program Year-End Evaluation Report is due by Close of Business on Monday, October 31, 2016. Please mail or hand deliver a hard copy and a CDROM of your completed report to:
