Qualifications Assessment Questionnaire
The California civil service selection system is merit-based, and eligibility for appointment is established through a formal examination process. This Heavy Truck Driver examination consists of a Qualifications Assessment Questionnaire that will be used to evaluate your experience, and education. The questionnaire is the entire examination and will account for 100% of your score. Candidates will receive notification of their score by mail.

- Completed Conditions of Employment (Form 631) (PDF)
- Evidence of valid driver license, operation of any combination of vehicles classified as Class A license (front and back copy of license)
