Dear Select Private School Owners and Administrators:

2021–22 Private School Affidavit Filing Reminder

The filing of the Private School Affidavit (PSA) is an annual requirement of all private schools offering elementary and high school level instruction in California. Per California Education Code Section 33190, an affidavit shall be filed with the California Department of Education (CDE) between October 1 and 15 of each year. Our records indicate that you filed an affidavit last year and have not yet filed an affidavit for the 2021–22 school year.Should you plan to continue to offer elementary and/or high school level private school instruction, please file a 2021–22 Affidavit as soon as possible.

If you have already filed an Affidavit, or are no longer operating a private school in California, please disregard this notice.

To file an Affidavit, please visit the CDE Private School Affidavit 2021–22 filing system at https://www3.cde.ca.gov/psa. Please note that you will need your prior year’s confirmation code to access the system and file your affidavit. For your convenience, your confirmation code is provided below. If you filed more than one Affidavit and need additional confirmation codes, you may use the Affidavit filing system to retrieve all of your confirmation codes by providing your email address.
