Title: Student Checklist

This image is a sample checklist on which a student will circle the answer to a question. The student may circle either a happy face or a sad face for each of five images related to proper techniques for lifting and carrying their backpacks.

There are three columns. The first column, which is on the left, has a different image in each row. The first row has a picture of a bathroom scale and the words, “Too Heavy? (8 to 10 pounds)?” The second row has a photograph of a small child bending both knees to lift his backpack and the words, “Two Bent Knees?” Third row has a photograph of a small child using both hands to pick up his backpack and the words, “Two Hands?” The fourth row has a photograph of a small child wearing both shoulder straps, one strap over each shoulder, and the words, “Two Straps?” The fifth row has a photograph of a small child standing with the backpack positioned close to his body and resting in the middle of his back and the words, “Too Low?”

The image in each row of the second column is a happy face. The image in each row of the third column is a sad face. These are the answer options the student will circle.
