Confirmation Code for Filing the 2023-24 PSA

This message is a follow up to the Annual Private School Affidavit (PSA) Filing Reminder Notice sent earlier today. As mentioned in that notice, previous year PSA confirmation codes, which are needed to prepopulate last year’s affidavit filing information would be provided in a separate email. Below is your previous year’s confirmation code.

CDS Code: (NOTE: If you have 5 or fewer students enrolled, you will not have a CDS Code assigned.)

Prior Year Confirmation Code:

If you filed a PSA for more than one school, only one code will be included in this email. To retrieve the confirmation codes for all associated schools, you may provide the email address associated with last year’s filing on the Retrieve Previous Year Affidavit screen (third step in filing a PSA if you have selected to file using last year’s information).
