Private School Designated Low-Income

Private School Designated Low-Income School List (DLISL) application information for fiscal year 2012-13. This information may also apply to other types of non-public schools.

Application Information

Please submit the following data on agency letterhead:

  1. School name
  2. Address
  3. Type of school - private profit or nonprofit
  4. CDS code - county, district, school code
  5. Grades - the grade span of students being taught at the school
  6. School district that the school is located in
  7. Enrollment data - time frame (as of October 2011)
  8. National School Lunch Program data
    1. Time frame (as of October 2011)
    2. Number of students eligible for free meals
    3. Number of students eligible for reduced-price meals
  9. Signature of Principal or Executive Director
  10. Submit original letter

A determination will then be made on the school's eligibility for placement on the designated low-income list for fiscal year 2012-13. The October 2011 data are used to determine eligibility for fiscal year 2012-13. These data must be submitted annually. Please mail the above information to:
