To all Prospective Contractors:
ADDENDUM #1–California Assessment System Request for Qualifications Stage One – CN220002
This addendum hereby revises RFQ CN220002, as follows:
Page 11, Section 4. General Submission Information
Add to the end of the paragraph under “Intent to Submit”:
Only the primary potential contractor is required to submit Attachment 1, Stage One: Intent to Submit form. Potential contractors who pass Stage One will be contacted via an email sent to the name listed on the Intent to Submit form.
Page 12, Section 5. Request for Qualifications (Stage One) Submission Specifications
Add to the end of the paragraph under “Submission Requirements”:
Appendices or other supplemental documentation are not to be included in the RFQ Stage One submission for each qualification. All forms in Stage One require a wet original signature. Digital signatures will not be accepted in Stage One.
Page 12, Section 5. Request for Qualifications (Stage One) Submission Specifications
Add as the third sentence of the paragraph under “Required Forms/Attachments”:
Graphics, such as diagrams and flowcharts, are not to be included in the narrative responses in Stage One.
Page 15, Section 5. Request for Qualifications (Stage One) Submission Specifications
Add as the third sentence to Section C under “Submission to CDE”:
All forms in Stage One require a wet original signature. Digital signatures will not be accepted in Stage One.
Page 18, Section 7. Qualifications (Scored)
Add as the last sentence to paragraph one:
Subcontractor references will be accepted for qualification responses where subcontractor scope and capabilities are predominantly featured.
Attachment 8: Subcontractors
Add the following paragraph under “Directions”:
Prime potential contractors cannot modify proposed subcontractors in Stages One and Two of the procurement process.
Attachment 12: Qualification Four—Scoring and Reporting Form
Replace the sixth bullet under “Directions” to the following:
Statistical reporting to track scoring progress.
