Decision to Offer Fluid Milk Substitutes in the Child Nutrition Programs (PDF)
The SNP-05 (Decision to Offer Fluid Milk Substitutes in the Child Nutrition Programs Form)
is for school food authorites to complete if they choose to offer a fluid milk substitute for medical or other special dietary needs to students at any of their school sites. The SNP-05 is also located in the Download Forms Section of the Child Nutrition Information and Payment System.

Parental or Guardian Consent Form to Release the Household Meal Application to the CalFresh Program (DOC)
Used by schools to obtain parental or guardian consent to release the household meal application to the local CalFresh office to initiate enrollment of the student's family in the CalFresh program.
