School Attendance Review Board Forms

The documents provide a variety of tools to help local school attendance review board (SARBs) promote positive attendance patterns.

The documents are suggested examples and may be modified as appropriate. Use of uniform attendance forms within the SARB area builds consistency and helps expedite processing of paperwork.

Although the California Education Code Section 48260.5 External link opens in new window or tab. requires very specific language for the first notification of truancy, most attendance documents can be modified to suit the population served by the SARB.

Note: Senate Bil 691, Statutes of 2024 External link opens in new window or tab., amended the initial truancy notice mandate and the habitual truancy notice mandate to state that districts may use the most cost-effective method possible for notification, which may include electronic mail or a telephone call.
