CAPSDAC Code Sets (XLSX) This document contains the current code values referenced in the CAPSDAC Data Domains, Fields, and Certification Frequency for the coded value data elements.

CAPSDAC Team Template and
Responsible, Accountable, Consulted, and Informed (RACI) Chart (XLSX)

 The Team Template and RACI Chart is a project management tool that suggests who fills what role for each task involved in developing a CAPSDAC Data Team and local CAPSDAC Data governance policy.
CAPSDAC 2.0 File Upload Template (XLSX) Provides sample templates and detailed instructions for preparing and uploading CSV files to the CAPSDAC 2.0 system. Includes built-in field validation, required/optional field indicators, and links to the Code Sets used in CAPSDAC data reporting. Supports LEAs and agencies in correctly formatting and submitting data during the certification process
