Survey Instructions

California State Preschool Program (CSPP) contractors who received written approval of CSPP Neighborhood School Eligibility from the California Department of Education (CDE) in 2024–25 must complete this annual reverification survey to determine CSPP neighborhood school eligibility for 2025–26. The survey must be completed separately for each previously approved CSPP site seeking to reverify, and all questions are required.

The survey asks for:

  • Agency legal name
  • Vendor number
  • Site name (as it appears in the Child Development Management Information System [CDMIS])
  • Qualified free or reduced-price meal (FRPM) school previously used to establish eligibility and its school district
  • Information regarding any changes to the school’s address and/or boundaries

Additionally, contractors will need to identify if the school is on the 2025–26 List of Eligible Public Elementary Schools, posted to the CSPP Neighborhood School Site Verification web page.

  • Did the approved CSPP Neighborhood School Site address change since it was last approved?
  • What was the name of the qualified FRPM elementary school that served as the basis for approval for the 2024–25 program year?
  • What school district is [qualified FRPM elementary school] in?
  • Is [qualified FRPM elementary school] in [school district] on the List of Eligible Public Elementary Schools released for the program year 2025–26?
  • Has [qualified FRPM elementary school] in [school district] address changed?
  • Have the boundaries of [qualified FRPM elementary school] in [school district] changed?

Note: If after completing the survey a site is approved, the agency will be asked to complete a certification statement.
