  • Accommodating Special Dietary Needs Quick Reference Guide found in the Download Forms section of the Child Nutrition Information and Payment System (CNIPS)

  • SNP 05 Notification of Decision to Offer Fluid Milk Substitutes in the CDE School Nutrition Programs (SNP) form found on the SNP Forms web page, as well as in the Download Forms section of CNIPS

  • SNP 26 Parental Request for Fluid Milk Substitution for School-Age Children form found in the Download Forms section of CNIPS

  • CNP 925 Medical Statement to Request Special Meals and/or Accommodations form found on the CDE School Nutrition Programs Forms web page, as well as in the Download Forms section of CNIPS

These updates clarify the changes to the fluid milk requirements in the National School Lunch Program.
