Description
Each subsidized child care services contractor who maintains an agreement with the California Department of Education (CDE) must regularly submit information about their program operations. These reports permit various CDE offices to determine the number and types of families served, monitor program costs, or assess agency compliance with contract terms.
Schedule of Reports
The following tables list each mandatory report by month and the due date. A number of reports are program type specific.
Use the letter designation within the Report Type column to determine which of the following offices or individuals to contact for assistance with report preparation.

CDD-801B, Monthly Sample Report
On approximately the seventh day of each month, a minimum of 200 families are selected for additional data collection beyond the CDD-801A, Monthly Child Care Population Information Collection Report. An agency with one or more of the selected families will receive a letter notifying them of the child and family information that must be entered into the CDD-801B, Monthly Sample Report through the Child Care Data Collection System within 14 days following the notice.
